Community-based rehabilitation services
Noble Intent offers Community Based Rehabilitation Services to our individuals. This is an individualized program that is focused on promoting recovery and improving the quality of life for our children and adolescents with mental health challenges.
Services are provided in the individual's home or in the community and focus on symptoms management, coping skills, communication skills, problem-solving skills, social skills, community living skills, and crisis support.
Each individual will work directly with our staff to identify their personal strengths, resources, and treatment goals. Our staff will assist each individual in meeting their treatment goals, learning healthy skills, and building supports that will help them lead happy and healthy lives.
We feel this is a necessary service in helping individuals learn to implement necessary skills in managing their symptoms independently in the community and at home.
Noble Intent offers and facilitates different therapy groups that will address and target the needs of individuals referred and screened. Groups can act as a support network and a sounding board. Other members of the group often help individuals come up with specific ideas for improving a difficult situation or life challenge and hold others accountable along the way.
Regularly talking and listening to others also helps someone put their own problems in perspective. Oftentimes, individuals may feel like they are the only ones struggling — but they’re not. It can be a relief to hear others discuss what they're going through, and realize they’re not alone.
Diversity is another important benefit of group therapy. People have different personalities and backgrounds, and they look at situations in different ways. By seeing how other people tackle problems and make positive changes, individuals can discover a whole range of strategies for facing their own concerns.

Noble Intent now offers Targeted Care Coordination (TCC) to our clients. TCC includes the assistance and coordination of the multidisciplinary Child and Family Team (CFT) meetings to design an outcome-based, strength-focused Person-Centered Service Plan (PCSP). This plan includes the information from the CANS and Comprehensive Diagnostic Assessment as well as the input of the child receiving services, their parents/guardians, and their care team.
The Person-Centered Plan encompasses the child's treatment goals, needs, strengths, a risk management plan, and a transition plan. Our Targeted Care Coordinators will ensure that all goals, needs, services, goods, and supports are met within the scope of that individual's personalized plan. TCC provides the level of care and treatment that each client and their family should receive.
TCC is offered to children and their families that have traditional Medicaid or are enrolled in the YES Program. To determine one's eligibility for the YES program, call an independent assessor at Liberty Healthcare at 1-877-305-3469.
