A TEAM APPROACH

Targeted care coordination (TCC) is a service available to youth ages 18 and under who have a significant mental health diagnosis. TCC facilitates the collaboration between clients, their professional supports, and community supports. This collaborative process allows all entities to coordinate the client's care and services. In the TCC process, a  person centered service plan is created with the team to identify current functioning, needs, strengths, and goals. TCC meetings occur monthly to allow for the continual reassessment of the client's person centered care plan. 

A PERSON-CENTERED CARE PLAN

The Person-Centered Plan encompasses the child's treatment goals, needs, strengths, a risk management plan, and a transition plan. Our Targeted Care Coordinators will ensure that all goals, needs, services, goods, and supports are met within the scope of that individual's personalized plan. TCC provides the level of care and treatment that each client and their family should receive.

TCC is offered to children and their families that have traditional Medicaid or are enrolled in the YES Program. To determine one's eligibility for the YES program, call an independent assessor at Liberty Healthcare at 1-877-305-3469.
